Group Homes & Corporate Adult Foster Care

Group Residential Housing - GRH
GRH is a state- funded program that provides at a minimum, room and board for adults who live in certain authorized group living arrangements. It is a program for adults with disabilities/disabling conditions who can’t support themselves. The goal of GRH is to prevent or reduce homelessness or institutionalization. GRH is unique in that the income supplement is not paid directly to the recipient.

Lutheran Social Service of Minnesota
Throughout Minnesota, we have licensed group homes and shelters which offer a secure, supportive and safe environment for children who are experiencing difficulties at home, in school, or in the community. They are staffed 24 hours a day, 7 days a week. The staff are well educated and very committed to their work with children in crisis.

Sanford Health Pathfinders Children's Treatment Center
Sanford Health Pathfinders Children’s Treatment Center is a place for children and families who need help finding the path to healthy emotional well-being. At our center, we welcome boys and girls age nine to 17 who are experiencing difficulties in school, home or the community. Patients may benefit from staying at our 14-bed residential mental health treatment facility where they will receive intensive therapy. In partnership with mental health professionals at Sanford Thief River Falls Medical Center, we offer the following inpatient and outpatient services: Diagnostic assessment, Psychological assessment, Home-based therapy and counseling, Respite care, Emergency shelter and Referrals and community collaboration.

Sanford Health Northern Lights Community Residence
Sanford Health Northern Lights Community Residence offers both Intensive Residential Treatment and crisis stabilization services that are fully supported by mental health experts at Sanford Health Thief River Falls Medical Center. Intensive Residential Treatment Services (IRTS) are available to help residents, care focuses around the following: individualized assessment and treatment planning, crisis assistance, development of health care directives and crisis prevention plans, nursing services, inter-agency case coordination and transition and discharge planning.
The facility also offers: integrated dual diagnosis treatment, illness management, opportunity for work experience at Northern Lights Book Store, family education and coordination with medical, outpatient mental health, chemical dependency and aftercare services. The Northern Lights facility has capacity to care for 15 adults at a time. Each resident is welcome to stay for up to 90 days.
Individuals who are eligible for services include any adult who is experiencing a psychiatric or mental health crisis. Crisis Stabilization services available include: crisis relapse recovery and prevention planning services, illness management and recovery services, individualized crisis stabilization assessment, treatment and discharge planning, supportive and short-term problem-solving counseling, medication monitoring, skills training and collaboration and coordination with agencies, providers, family and community.
