Personal care assistance services help a person with day-to-day activities in their home and community. PCAs help people with activities of daily living, health-related procedures and tasks, observation and redirection of behaviors and instrumental activities of daily living for adults.

Prenatal Education
A public health nurse can work with you one-on-one in the community or in your home to provide education for a healthy pregnancy. Education will include physical and emotional changes during pregnancy, how to prepare for labor and birth, breastfeeding, newborn care, and growth and development.

Post-partum/Newborn Visits
A public health nurse will visit you after you return home with your baby. The nurse will assess baby’s growth and development, answer any questions you may have and will connect you to community resources available to you.

Help Me Grow
Program designed for children with special needs such as speech delays, movement problems or learning disabilities. Provide coordination of services such as physical therapy, speech therapy, early childhood special education or any other needed services to help children develop.

Immunizations are available in the office by appointment to children 0-18 who are eligible for the Minnesota Vaccines for Children Program. Eligibility includes those who are uninsured, have insurance through a state-funded plan or privately insured but vaccines are not covered.

Supplemental Nutrition Assistance Program (SNAP)
SNAP, formerly known as Food Stamps, helps Minnesotans with low incomes get the food they need for nutritious and well-balanced meals. The program provides support to help stretch your household food budget. It is not intended to meet all of your household’s food needs. It is a supplement. If you are approved for SNAP, you can use your benefits at many stores, farmers markets and senior dining sites. Electronic Benefits Transfer (EBT) debit cards are used to make food purchases.
